Grip strength is a biomarker, not a party trick

Hand strength predicts a startling range of outcomes it has no business predicting. Here is why — and why our dead hang test is an imperfect stand-in for it.

Squeeze a dynamometer as hard as you can. The number that comes out correlates with all-cause mortality, cardiovascular events, length of hospital stay after surgery, recovery from illness, and the likelihood of losing independence later in life.

That is a lot of freight for one hand.

Why it works

Bohannon's 2019 review makes the case that grip strength functions as an indispensable biomarker in older adults, and the reason is not that hands are special. It is that grip is a cheap, low-variance readout of something that is otherwise expensive to measure: total muscle mass and neuromuscular function.

Whole-body strength is tedious to assess and requires equipment and technique. Grip requires neither, has excellent test–retest reliability, and moves in step with the systemic thing you actually care about. When people lose muscle across the body, grip goes with it — early, and measurably.

Pratt and colleagues published normative grip data from 9,431 participants in the GenoFit study, which is what a real normative dataset looks like: large, stratified by age and sex, and honest about its spread.

The Indian caveat, stated plainly

Indian grip strength data run lower than American and most European reference values. If you measure yourself against a Western normative table you will look worse than you are relative to your own population.

That does not make the measurement useless. It makes the comparison less meaningful and your own trend over time more meaningful. Which is broadly true of everything on this site.

Why we use a dead hang instead

Our screen does not use a dynamometer, because you do not own one. We use a dead hang: arms fully extended, straight spine, hold the bar, and we time it.

It is a proxy, and it is worth being direct about the ways it is a worse measurement than the real thing.

If you want the actual measurement, a hand dynamometer costs very little and gives you a number you can compare against published norms directly. That is a genuine upgrade over what we can do in a browser, and we would rather say so than pretend our proxy is the real thing.

What grip is telling you

Treat a poor dead hang as a prompt to look at two patterns rather than as a hand problem.

Pull. The most under-trained pattern in most adults. Nobody in a sedentary job pulls anything heavy, and the capacity goes quietly. Lat pulldowns, rows, and eventually pull-ups.

Carry. Loaded carries are the most direct training for grip endurance and they transfer immediately to real life — the two heavy bags up three flights when the lift is out. A farmer's carry heavy enough that the last few steps are a real decision is worth more here than any amount of forearm work.

The honest summary

Grip strength is one of the best-supported simple biomarkers in the ageing literature. Our version of it is a reasonable field approximation with a known flaw. Both of those things are true, and you should hold your dead hang time with that in mind — as a number to beat in four weeks, rather than as a verdict about your health.
